Each member of Zócalo Health is paired with a promotor de salud (community health worker) to establish a relationship to better understand members’ goals and connect them to a Zócalo Health physician. Our team of physicians, hired from the community, focus on prevention, primary care, behavioral health, and traditional practices that work together to support our members’ wellness. Operating in the states of California Texas, and Washington, Zócalo Health offers affordable and convenient care memberships that provide same-day access to culturally-aligned providers.

Job Summary

Zócalo Health is looking for a promotor de salud who will work directly with our members to help them navigate their Zócalo and local health care ecosystems to promote mental, physical, and social wellness. You’ll be at the front end of an innovative program which incorporates community health workers and virtual care in the context of a primary care practice.

As a promotor de salud, you will assess, define, and implement Zócalo Health care plans individually tailored for each member. In partnership with the care team, you will work on increasing the patient’s knowledge of health and health risks, improving current health practices, promoting Zócalo Health services, and supporting preventive activities with community collaboration. Finally, you will develop and adapt our health promotion material according to your analyses to ensure relevance, adequacy, and comprehension.

Responsibilities

• Engage the community, actively leading membership enrollment, and responding to inquiries from prospective members to guide them through the enrollment process.
• Actively engage with a panel of assigned Zócalo Health members, to help them increase their health literacy.
• Collaborate with a multidisciplinary team to care for a panel of patients, ensuring they are up to date on preventive measures and their care plans.
• Provide care navigation, assisting members with: appointment logistics, prescription drug support, lab support, referral coordination, care plan adherence, sharing information with the care team, and providing resources.
• Co-develop care plans that address both clinical and social needs, understanding social determinants of health that are affecting the patient.
• Screen individuals for Social Determinants of Health (SDoH) and refer them to community resources for services such as financial assistance, legal aid, housing, food, or education.
• Coach patients using an action-planning model based on motivational interviewing techniques. Document services provided in client records and required databases.
• Communicate with patients via phone, video conference, and text messaging, involving other members of the multidisciplinary care team as needed.
• Provide health education on acute and chronic disease management through various services, including presentations, workshops, trainings, and small group discussions.
• Build relationships with community leaders and organizations, and serve as a liaison with local community based organizations.
• Participate in community events and activities. Ensure regular collaboration and coordination with the medical team to align medical activities with member health programs.
• Report member difficulties and progress, during medical rounds, and help make changes that allow for the member to participate in their care plan.
• Participate in the daily operations of a primary care practice, such as answering calls, handling requests, and maintaining the clinical space.
• Depending on the needs, your responsibilities may be adapted and evolve accordingly. You will receive technical support from other Zócalo team members, including comprehensive guidelines and protocols.
